How to Complete Forms

Follow these simple steps to ensure your forms are completed correctly and your child can participate in all Pathfinder activities safely.

Step 1

Click on the Permission Slip and Medical Waiver to download them to your device.

Step 2

Fill out all sections completely. Incomplete forms cannot be accepted for safety reasons.

Step 3

Print the completed forms and obtain all required signatures from parents/guardians.

Step 4

Bring completed forms to your first meeting or email them to our club director.
